Proportion of eligible patients (composite)
The percentage of eligible patients (the denominator) who meet ALL of these criteria: Blood pressure measured (within 1 year) AND Blood pressure at target (within 1 year). Each patient is counted once.

What this indicator measures— Name, alignment, goal, rationale.

- AlignmentField 2 — Alignment
- Hypertension Canada Guidelines
- Ontario Health MyPractice Primary Care indicator catalogue
- Strategic goalField 3 — Strategic goal
- Ensure patients with hypertension have their blood pressure both monitored and controlled to the guideline target, reducing cardiovascular and renal risk.
- RationaleField 5 — Rationale
- Control requires BOTH measurement and a result at target — measuring without achieving control, or an old at-target reading with no recent measurement, is not adequate control. The AND combiner expresses this honestly.
How it's computed— Scoring method, numerator, denominator, inclusions, exclusions.
- NumeratorField 6 — Numerator
- Active rostered patients in the denominator with a documented BP measurement within 1 year AND a documented at-target measurement within 1 year of the reporting-period end.
- DenominatorField 7 — Denominator
Active rostered patients with hypertension, age 18+ at reporting-period end.
- Sex eligibility: F, M
- Age range: 18–120
- Active as of reporting-period end: yes
- ExclusionsField 9 — Exclusions
- Patients in palliative care.
- Patients deceased before the reporting-period end date.
